Every student athlete must be cleared by the athletic training staff and team physicians before participating in varsity sports, including Cheerleading/Spirit Squad and Dance team, at Washington State. This page outlines all the paperwork that must be done before going to pre-participation physicals. Your sport coach will inform you about dates and times for your pre-participation physical.

All WSU Student-Athletes Checklist (If under 18 your parent(s) or guardian will have to sign most of these forms.)

• _____ General Information: fully completed
• _____ Insurance form (2pgs): fully completed and signed (primary, secondary and dental). ALL students are encouraged to have insurance. WSU offers a
               student medical plan that you can purchase. All International students with non-immigrant status (except Canadians), who are enrolled for one credit or
               more, are automatically enrolled in this plan. (http://studentinsurance.wsu.edu/general-plan)
• _____ Insurance card(s): copy of front and back (primary and secondary)
• _____ Dental card: copy of front and back
• _____ Assumption of Risk and Release of Liability: print, sign and return
• _____ Disclaimer & Release of Medical Information: print, sign and return
• _____ Statement for Nutritional Supplements: print, sign and return
• _____ Understanding Concussions: print, sign and return

Freshman/Transfer Student-Athletes

• _____ New/Transfer Student-Athlete Medical Examination: fully completed - WSU Athletic Training will provide the actual physical when you arrive in Pullman;
               fill out, sign and return
• _____ Sickle Cell Trait Testing: fully completed, sign and return. You will not be allowed to participate until we have the results - which takes 3-5 days. You can
               have this test done in advance by your physician and have the results forwarded to us.

Returning Student-Athletes

• _____ Returning Student-Athlete Medical Examination: fully completed, sign and return

WSU Athletic Training must have this information as soon as possible. You will not be able to practice with your team until this information has been received and completed!
